Clinic in DHCC or Mainland Dubai: Which Should You Choose?
DHCC vs mainland clinic in Dubai: regulators after the 2024 laws, ownership, licence fees, tax, location, insurance and moving later. A decision guide for 2026.
Choose Dubai Healthcare City (DHCC) if you want a free zone company (FZ-LLC or branch) in a medical cluster, with free zone ownership rules and a fixed commercial licence fee (AED 15,000 a year for most outpatient activities). Choose the mainland if you want to be in a residential or business district near your patients, with a trade licence from Dubai Economy and Tourism. Since the 2024 Dubai health laws, both routes need DHA licensing for the health activity. So the decision comes down to location, ownership structure and running costs, not regulatory shortcuts.

What changed in 2024 and 2025
For years, DHCC ran its own regulator, DHCR, separate from DHA. That is the main reason older guides describe DHCC as a different licensing world. Three instruments changed this:
- Law No. 16 of 2024 (4 September 2024). Article 20 says nobody may carry on a healthcare activity in DHCC without DHA licences and permits, in addition to DHCA approvals.
- Executive Council Resolution No. 49 of 2024 applies to all facilities practising health activities in Dubai, "including in Special Development Zones and free zones".
- DHCA Rule No. 1 of 2025 (effective 2 May 2025) lists DHA as the external approval for clinical activities and requires a Clinical Operating Permit before a facility starts operating.
The day-to-day process for professional licences inside DHCC is still described differently by different sources. Ask DHCA in writing which portal your clinicians should use before they apply. Background: Dubai Healthcare City (DHCC) guide.
Ownership
DHCC. Law No. 16 of 2024 lets zone companies be owned by one or more people or companies, whether or not they are UAE nationals. Allowed structures are an FZ-LLC or a branch of a foreign or UAE company.
Mainland. Since 1 June 2021, DET has allowed 100% foreign ownership for more than 1,000 commercial and industrial activities, with no Emirati partner required, according to the Dubai Media Office. Gulf News lists "health and social work" among the industries on Dubai's list. Check your exact activity on Invest in Dubai before you commit.
On both routes, DHA cares that the facility has a licensed medical director and enough licensed staff. If the owner is not a clinician, see can a non-doctor own a clinic in the UAE.
Who you can treat, and where
Neither licence limits which patients you see. A DHCC clinic treats Dubai residents, visitors and insured patients like any other DHA-licensed facility, and the Dubai health insurance law applies inside the zone. The real limit is where you operate. A DHCA licence covers premises in DHCC, and the facility name must state on ads, invoices and contracts that it is a free zone establishment. To open a second branch in, say, Dubai Marina or Mirdif, you need a mainland set-up.
Moving later is possible but not quick. DHA's manual says a free zone facility moving to the mainland needs a no objection certificate from the free zone authority and must register as a new entity on the mainland, with a new facility licence process.
What drives cost on each route
| Cost driver | DHCC | Mainland |
|---|---|---|
| Commercial licence | AED 15,000 a year for most outpatient activities, published in Rule No. 1 | Varies by activity. DET initial approval AED 120 and trade name AED 620, per Khaleej Times |
| Rent | Limited buildings and landlords inside the zone | Wide choice; varies by district, floor and footfall |
| Share capital | 10,000 minimum for most outpatient FZ-LLCs | Depends on legal form |
| DHA facility and professional fees | Same on both routes: see cost to open a clinic in Dubai | |
| Fit-out | Driven by DHA Health Facility Guidelines and your room schedule on both routes | |
| Visas | Through DHCA | Through MOHRE and GDRFA |
When DHCC makes sense
- You want a medical-cluster address next to hospitals, labs and referral partners.
- You are a foreign group that prefers a free zone company or a branch of your existing company.
- Your patients travel to you: specialist, tertiary or medical tourism services.
- You value the zone's tax regime and a published licence fee for planning.
When mainland makes sense
- Your patients choose by proximity: GP, family, paediatric, dental or walk-in services.
- You plan several branches across Dubai.
- You need ground-floor visibility and parking in a residential area.
- Your target district has more demand than supply. That is a bigger revenue driver than the licence type.
Questions to ask before you sign a lease
- Which DHA facility category will this be? (See licence types.)
- Does the unit fit the DHA room schedule, checked by a DHA-prequalified designer?
- In DHCC: which DHCA permits, which DHA approvals, and which portal for staff licensing?
- On the mainland: is 100% foreign ownership available for your activity code?
- Which insurers' networks are short of your specialty in that area?
Marketing before you open
Create your Google Business Profile with a future opening date: Google shows it 90 days before you open. Build the landing page and a pre-opening Google Ads campaign now, but switch ads on only once the licence is active. DHA bars advertising health services while the licence is inactive, and ads need DHA-compliant content and approval. Get a free launch plan, or read how to start Google Ads for a clinic.
Frequently asked questions
Do DHCC clinics still need a DHA licence?
Yes, for the health activity. Law No. 16 of 2024 requires DHA licences and permits for healthcare activities in DHCC, in addition to DHCA's commercial licence and permits.
Is it cheaper to open a clinic in DHCC or on the mainland?
It depends mostly on rent and fit-out, which vary on both routes. DHCC publishes an AED 15,000 commercial licence for most outpatient activities. DHA fees apply either way.
Can a DHCC clinic treat patients with insurance?
Yes. Dubai's health insurance law applies inside DHCC under Resolution No. 8 of 2022. You still contract with each insurer or TPA separately.
Can I move my clinic from DHCC to the mainland later?
Yes, but it is treated as a new entity. You need a no objection certificate from the free zone authority and a new mainland registration and facility licence.
Can foreigners own 100% of a mainland clinic in Dubai?
DET allows full foreign ownership for more than 1,000 activities, and health activities appear on Dubai's list. Confirm your specific activity code on Invest in Dubai.
