Google Ads Demand Gen Campaigns: A Guide for Clinics
Google Ads Demand Gen explained for clinics: YouTube, Discover, Gmail and Maps placements, formats, health audience limits, budget rules and a setup checklist.
Demand Gen is a Google Ads campaign type for image and video ads on YouTube (including Shorts), Discover, Gmail, Google Maps and the Display Network. It reaches people before they search, like Instagram ads do, rather than people typing "dentist near me". For clinics it works best for elective, visual treatments and for building demand that Search later captures. Health is a sensitive category, so customer lists, lookalike segments and your own data segments cannot be used to promote treatments for health conditions.

What Demand Gen is
Google describes Demand Gen as campaigns that capture engagement and action across YouTube, Discover, Gmail, Maps and the Google Display Network, using AI to show your best visuals to people who may be interested but are not searching yet (Google Ads Help). Google also says Display campaigns are moving into Demand Gen, with voluntary migration starting in June 2026.
Think of it as Google's answer to Meta ads. Search ads meet intent. Demand Gen creates it.
Where the ads show and which formats you need
| Placement | Formats |
|---|---|
| YouTube: in-stream, in-feed, Shorts | Video (landscape, square, vertical) |
| Discover feed and Gmail | Single image, carousel (2 to 10 cards) |
| Google Display Network (optional) | Image |
| Google Maps | Only serves when a non-affiliated location asset is on the account |
Image ratios: landscape 1.91:1, square 1:1, portrait 4:5, logo 1:1. Google says campaigns can be image-only or video-only, but using both usually performs better. Channel controls let you keep "All Google channels" (Google's recommendation) or pick channels per ad group (channel controls).
The health restriction most clinics miss
Google treats health as a sensitive interest category in personalised advertising. Its policy covers physical or mental health conditions, including chronic conditions, sexual health, pregnancy and infertility. When you promote in a sensitive category, Customer Match, your data segments (website visitor lists), audience expansion and lookalike segments are not supported (Health in personalised advertising). You can still use Google's predefined audiences: in-market, affinity, life events, demographics and location.
The Demand Gen help page adds that ads in sensitive categories are generally restricted to YouTube and must use predefined Google audiences. So a fertility, mental health or weight-loss clinic should expect fewer placements and no remarketing. A salon, a whitening offer or a general check-up campaign has more room. Check each campaign against the policy before you build audiences.
When Demand Gen makes sense for a clinic
- Elective, visual treatments: aesthetics, laser hair removal, smile makeovers, hair transplant consultations. People need to see results and the clinic before they search.
- A new branch or new service that nobody searches for by name yet.
- Video you already have: doctor explainers, clinic tours, patient-journey content (with written consent, per DHA rules).
- Feeding Search: a viewer who watched your implant explainer is more likely to click your Search ad later.
It makes less sense for urgent or need-based care (GP, urgent care, home nursing). Those patients search. Put that budget into Search first: see how to start Google Ads for a clinic.
Setup checklist
- Conversion tracking working with a Google tag. Google suggests lighter events can help, but for clinics count real leads (call, WhatsApp, form) as primary. See conversion tracking guide.
- Budget. With target CPA bidding, Google recommends a daily budget of at least 10 times your target CPA. If you want leads at AED 150, that is AED 1,500 a day. Most small clinics should start on Maximize Conversions or Maximize Clicks instead.
- Creative. At least one video in vertical and landscape, plus images in all three ratios. Fix any ad flagged "Eligible (limited)".
- Audiences within the health policy. In-market and life-event segments plus location. No uploaded patient lists for health-condition services.
- Copy that passes DHA. No "the best", "100%" or "guaranteed". Before-and-after images are a problem on Google: its clickbait policy bans "before and after" images promoting significant changes to the body. See before-and-after photos in clinic ads.
- Landing page built for cold traffic: who the treatment is for, the doctor, price guidance where allowed, and a WhatsApp button.

Frequently asked questions
What is Demand Gen in Google Ads?
A campaign type for image and video ads on YouTube, Discover, Gmail, Maps and the Display Network, aimed at people who are not actively searching yet.
Is Demand Gen replacing Display campaigns?
Google says Display campaigns are moving to Demand Gen, with voluntary migration starting in June 2026.
Can clinics use remarketing in Demand Gen?
Not when promoting treatments for health conditions. Google's health policy blocks your data segments, Customer Match and lookalikes in sensitive categories. Predefined Google audiences still work.
What budget does Demand Gen need?
With target CPA bidding, Google recommends a daily budget of at least 10 times the target CPA.
Is Demand Gen better than Meta ads for a clinic?
They do a similar job on different platforms. Many aesthetic clinics run both and judge them on booked consultations, not clicks.
