How to Choose a Medical Marketing Agency

What a medical marketing agency does, how agencies price (retainer, % of spend, per lead), 15 questions to ask before signing and red flags to avoid.

Guide · 18 Oct 2026 · Access One

Quick answer

A medical marketing agency gets patients for clinics, hospitals and health services through Google Ads, local SEO, social media, websites and tracking, while keeping ads inside medical advertising rules. They usually charge a monthly retainer, a percentage of ad spend, a fee per lead, or a mix. Hire one that lets you own every account, reports cost per booked patient, and can explain your regulator's ad rules without looking them up.

What a medical marketing agency actually does

A medical marketing agency (also called a medical advertising agency or healthcare marketing agency) is a marketing firm that works mainly or only with healthcare providers. The core services are the same as any digital agency. The difference is the rules and the patient journey.

ServiceWhat you should receiveWhat "medical" adds
Google Ads (PPC)Search campaigns per treatment, call and form tracking, weekly optimisationHealthcare policy limits, prescription drug terms, no Customer Match for health ads
Local SEOGoogle Business Profile management, reviews routine, location pagesPractitioner vs practice profiles, licence data consistency
Website and landing pagesFast pages per treatment with booking, prices, doctors, insuranceMedical review of claims, consent for photos, licence numbers
Content and SEOTreatment pages, FAQs, doctor biosWritten or reviewed by licensed clinicians, no guaranteed outcomes
Social media and Meta adsContent calendar, paid campaigns, lead formsBefore-and-after rules, influencer approvals, Meta health data limits
Tracking and reportingCalls, forms, WhatsApp, bookings by channelNo patient data sent to ad platforms
ReputationReview requests, repliesNo incentives, no patient details in replies

Why the "medical" part matters

Generalist agencies often write copy that is normal in retail and illegal in healthcare. In Dubai, the DHA medical advertising standard bans words like "the best", "unique", "100%" and "immediate results", requires written patient consent for photos and testimonials, and makes the clinic's medical director accountable for every ad that names the facility. The clinic carries the risk, not the agency.

Google adds its own limits. Its Healthcare and medicines policy restricts prescription drug terms, and since the October 2025 update promotional use of those terms is allowed only in the US, Canada and New Zealand. An agency that does not know this will keep writing "Botox" ads for a Dubai clinic and wonder why they are disapproved. More on this in Google Ads for dentists and clinics.

Pricing models compared

Agencies price in five common ways. The figures below are worked examples to show how each model behaves, not market rates; fees vary widely by country, scope and agency size.

ModelHow it worksExampleGood forWatch out for
Flat monthly retainerFixed fee for a defined scope$2,000/month for Google Ads + reportingPredictable budgetsVague scope, "unlimited" promises
% of ad spendFee scales with media budget15% of $10,000 = $1,500Larger, growing budgetsIncentive to spend more, not spend better
Retainer + % hybridMinimum fee plus % above a threshold$1,000 + 10% of spend over $5,000Mid-size clinicsCheck the maths at your real budget
Pay per leadFee per enquiry delivered$40 per qualified form or callTesting an agencyLow-quality leads, agency owns the data
Project feeOne-off price for a build or auditWebsite rebuild, account auditDefined deliverablesOngoing work not included

Do the total cost maths. If your ad spend is $5,000 and the fee is $2,000, your real cost per click is 40% higher than the platform shows. Divide total monthly cost (media + fee + tools) by booked patients to get the only number that matters: cost per booked patient.

What a clean contract includes

  • Scope by deliverable (campaigns, pages, posts, reports per month).
  • Ad spend paid directly by you to Google and Meta, not marked up through the agency.
  • Notice period of 30 days, not 12-month lock-ins.
  • All accounts, creative files, landing pages and data are yours on exit.
  • A named person doing the work, and who replaces them.
  • Data processing terms covering any patient data the agency touches.

15 questions to ask before you sign

  1. Who will own the Google Ads, Google Analytics, Google Business Profile, Meta Business and website accounts?
  2. Can I see two current healthcare clients' results as cost per booked patient, with permission?
  3. How many clinics in my specialty and city do you work with? (Competing clients in one area bid against each other.)
  4. Who writes the ad copy, and who checks it against DHA, DoH, MOHAP or my local regulator's rules?
  5. How do you handle Google's healthcare policy and prescription drug terms in my country?
  6. How will you track phone calls, WhatsApp and form bookings?
  7. Can you import booked appointments from our practice system into Google Ads? (See conversion tracking.)
  8. Which bidding strategy will you start with, and when will you switch? (See Smart Bidding.)
  9. What will you do in the first 30, 60 and 90 days?
  10. What does the weekly or monthly report contain? Ask for a sample.
  11. Is ad spend billed by you or by Google and Meta directly?
  12. What is the notice period and what do I get when I leave?
  13. Who actually works on my account, and how many accounts do they manage?
  14. How do you collect reviews without breaking Google's rules?
  15. What will you need from my front desk and doctors each week?

Red flags

  • Guaranteed rankings or "page one in 30 days". Google states there is no way to request or pay for a better local ranking, and no honest SEO can guarantee positions.
  • The agency owns your Google Ads account. Google lets a manager account own a client account, and recommends granting ownership only when truly needed. If you leave, you could lose years of data. Insist you are an admin on every account.
  • Reports full of impressions and clicks, no bookings.
  • Review schemes: discounts for reviews, asking only happy patients, or staff writing reviews. Google's review policy bans all three, and the US FTC rule on fake reviews (effective 2024) allows civil penalties of up to $51,744 per violation.
  • Ad copy with "best", "guaranteed" or "painless" in a market where the regulator bans those claims.
  • Remarketing lists or Customer Match for health services. Google's personalised ads policy blocks advertiser-curated audiences for health ads.
  • Patient names or conditions passed into tracking (URLs, conversion names, form data sent to Meta).
  • Long lock-in contracts with no performance exit.

Does a Google Partner badge mean anything?

It is a floor, not a guarantee. To earn the Google Partner badge, an agency must keep an optimisation score of at least 70% across its manager account, about $10,000 in ad spend over 90 days, and Google Ads certification for at least half of its strategists. Premier Partners are the top 3% in their country by spend growth, retention and other factors. A badge tells you the agency runs real budgets. It does not tell you they understand medical advertising rules, so still ask the questions above.

In-house, freelancer or agency?

OptionBest whenMain risk
In-house marketerGroup with 3+ clinics, lots of content and eventsOne person cannot be expert in ads, SEO, design and tracking
FreelancerSingle clinic, one channel (e.g. Google Ads only)Holidays, illness, no backup
Specialist agencyYou need several channels and compliance know-howJunior staff on your account, long contracts

Many clinics run a hybrid: an in-house coordinator who handles photos, reviews and the front desk, plus an agency for paid ads and technical SEO. Dental practices can find channel-specific advice in dental marketing and dental SEO.

How to run a fair 90-day trial

  • Agree the baseline: last 90 days of enquiries, bookings and spend.
  • Agree one target metric, such as cost per booked patient for implants.
  • Give the agency access to call recordings or a weekly booking export.
  • Meet every two weeks for the first two months.
  • At day 90, compare like for like, adjusting for seasonality (in the UAE, compare Ramadan and summer-travel months with the same months last year).

Frequently asked questions

How much does a medical marketing agency cost?

It depends on scope and market. Common models are a flat monthly retainer, a percentage of ad spend, a hybrid of both, or a fee per lead. Always add the fee to your ad spend and divide by booked patients to compare agencies fairly.

What is the difference between a medical marketing agency and a medical advertising agency?

The terms are used interchangeably. "Advertising agency" sometimes means a firm focused on paid media and creative, while "marketing agency" may also cover SEO, websites and reputation. Ask for the exact scope either way.

Should my agency own my Google Ads account?

No. You should create or own the account and give the agency access. Google allows manager accounts to own client accounts, but you risk losing data and history if the relationship ends.

Can an agency guarantee new patients?

No agency controls your front desk, pricing or reputation. Agencies can commit to activity, transparency and targets, but a guarantee of patient numbers or rankings is a warning sign.

Do medical marketing agencies need to know local regulations?

Yes. In Dubai, for example, the DHA standard bans exaggerated claims and controls before-and-after photos, and the clinic's medical director is accountable. Your agency should check every ad against these rules before it runs.
