Why are my Meta ads getting rejected for my clinic?

Why Facebook and Instagram reject clinic ads: personal attributes, negative self-image, 18+ rules and claims. Fixes with before-and-after copy examples.

Guide · 19 Oct 2026 · Access One

Quick answer

Clinic ads on Facebook and Instagram are usually rejected for one of four reasons: the copy implies the viewer has a health condition ("Struggling with hair loss?"), the image creates negative self-perception (weight-loss before-and-afters, pinched fat, zoomed-in body parts), the ad promotes a cosmetic or weight-loss service without an 18+ audience, or the landing page breaks a rule. Fix the creative, then request a review in Account Quality. Repeated rejections can restrict the ad account, so do not keep re-uploading the same ad.

The four rejection reasons we see most

1. "Personal attributes": your copy talks about the viewer's health

Meta's advertising standards ban ads that assert or imply personal attributes, including medical or physical and mental health conditions. Using "you" is allowed, but not to suggest or ask whether the viewer has a condition (explained here; policy on Meta Transparency Center).

2. "Personal health and appearance": the image

Meta does not allow ads that imply or try to create negative self-perception to promote health or appearance services. Its examples of non-compliant images include weight-loss before-and-after pictures, a person on a scale, zoomed-in abs and wrinkle close-ups (Meta policy). Close-ups of fat being pinched are a classic rejection. Show the clinic, the doctor, the device, or a confident result framed positively instead.

3. Wrong age targeting for cosmetic and weight-loss services

Meta's health and wellness rules require ads for cosmetic procedures, weight-loss products and services, and dietary supplements to target people aged 18 or over (summarised by Accelerated Digital Media; policy on Meta Transparency Center). Advantage+ audience settings can widen an audience, so check the minimum age control, not just the suggestion.

4. The landing page, the claims or prescription drug names

  • Claims to cure, guaranteed results, or "no side effects" get flagged, and in Dubai the DHA bans words such as "the best", "100%" and "miraculous" anyway.
  • Prescription drug and brand names (for example injectable brand names) trigger restricted-goods reviews.
  • Broken pages, pop-ups that block content, or a page that does not match the ad.

How to fix a rejected clinic ad, step by step

  1. Read the exact reason in Ads Manager or Account Quality. "Personal attributes", "Personal health and appearance" and "Health and wellness" need different fixes.
  2. Fix the creative, not just the wording around it. Rewrite second-person health questions as service statements. Replace body close-ups and weight-loss before-and-afters.
  3. Set the audience to 18+ for any aesthetic, cosmetic or weight-loss treatment.
  4. Check DHA compliance at the same time. No banned words, licensed titles only, consented and unedited patient images with a results-vary statement and risks. See before-and-after photo rules.
  5. Request a review only if you believe the decision is wrong. If the ad really broke the rule, edit and resubmit instead.
  6. Do not duplicate rejected ads into new ad sets. Repeated violations build up against the account and can lead to restrictions.
  7. Watch old ads too. Ads that were approved months ago can be reviewed again when edited or reused under current rules.

Rejections vs data restrictions

A rejected ad is different from a health data restriction. Since 2025 Meta has been placing some health and wellness advertisers in tiers that limit which website events can be used to optimise, for example blocking lower-funnel events (Search Engine Land). Your ads still run, but results get more expensive. Instant forms and WhatsApp campaigns are less affected. Give custom events neutral names that do not describe a health condition.

Write approval-safe clinic ads from the start

  • Talk about the service, the doctor and the outcome in neutral terms.
  • Use the clinic, team and equipment in visuals.
  • Keep before-and-after images for your website, where you control disclaimers, and only where the platform allows them.
  • Keep a pre-approved copy bank that the medical director has signed off.

Frequently asked questions

Can I use before-and-after photos in Meta ads for my clinic?

Weight-loss before-and-afters are listed as non-compliant by Meta. Other cosmetic before-and-afters are high risk and in Dubai must also meet DHA rules: same patient, same lens, no editing, results-vary statement and risks.

Why does Meta reject "Are you suffering from back pain?"

It implies the viewer has a health condition, which breaks Meta's personal attributes policy. Rewrite it as "Physiotherapy for back pain".

Will repeated rejections get my ad account banned?

Repeated policy violations can lead to account restrictions. Fix the cause before you upload more ads.

Do aesthetic clinic ads need 18+ targeting on Meta?

Yes. Cosmetic procedures and weight-loss services must target people aged 18 and over.
