Should I hire an agency or run Google Ads myself?
Agency or DIY Google Ads for a clinic? A quick decision table, a DIY checklist, what to demand from an agency, Google Partner rules and red flags.
Run Google Ads yourself if you have one location, one or two treatments, a modest budget and a few hours every week to learn and check the account. Hire an agency, ideally a healthcare specialist, once you advertise several treatments or branches, spend serious money each month, or cannot set up call, WhatsApp and booking tracking yourself. Either way, keep the account in your clinic's name and insist on seeing clicks, impressions and total cost. Google says those are your right.

The honest trade-off
Google Ads is not hard to switch on. It is hard to run well for a clinic. Three things make healthcare harder than most industries:
- Policy limits. No remarketing or patient-list targeting for health ads under Google's personalised advertising policy, and no prescription drug names in the UAE under the healthcare and medicines policy.
- Local rules. The DHA medical advertisement standard bans common ad phrases and requires medical director approval.
- Tracking. Most clinic bookings come by phone or WhatsApp. If you only track form fills, automated bidding optimises towards the wrong thing.
Decide in 2 minutes
| Your situation | Best option |
|---|---|
| One branch, 1 to 2 treatments, small budget, you enjoy the numbers | Do it yourself, with a one-off expert setup or audit |
| Several treatments, one branch, no time to check weekly | Agency or freelancer with healthcare experience |
| Several branches or specialties, larger monthly spend | Specialist healthcare agency |
| You have a marketing manager in-house | In-house management, agency for setup, tracking and quarterly review |
| Account was suspended or ads keep getting disapproved | Specialist help now |
If you run it yourself: the checklist
- Learn the basics. Google's own Skillshop training and the Google Ads Search certification are free.
- Set up conversion tracking for calls, forms and WhatsApp before you spend.
- Use Search only, a tight radius and staffed hours. Follow how to start Google Ads for a clinic.
- Block time every week: search terms report, negatives, budget pacing, call answering.
- Judge on cost per booking, not clicks. Understand cost per click and Quality Score.
- Be careful with Google's automatic recommendations. Some raise budgets or broaden targeting. Read each one before you apply it.
The hidden cost of doing it yourself is not the fee you save. It is wasted spend while you learn, and your hours away from patients and the business.
If you hire an agency: what to insist on
Google's advertiser guide to working with third parties is clear on your rights. Turn it into your checklist:
- Your own account. The account should be separate and set up for you. Create it under the clinic's Google account and give the agency access. A Google Ads account has one owner manager account, and you can remove an agency by unlinking it. Keep at least one admin user from the clinic.
- Full cost transparency. You have the right to see clicks, impressions and total ad cost. Ask for the agency fee in writing, separate from ad spend.
- Budget in the contract. Google advises getting the agreed budget in writing.
- Reporting on bookings. Monthly reports should show enquiries and bookings by treatment, not just clicks.
- Real Google Partner status. Click the badge: it should open a Google profile page. Know what it means. Google's Partner requirements are a 70% optimisation score, USD 10,000 spend across managed accounts over 90 days, and certified staff. Premier Partner is the top 3% of partners in a country. The badge proves skill and scale, not healthcare experience, so ask for clinic examples too.
- Healthcare knowledge. Ask how they handle remarketing limits, prescription drug terms and DHA wording. Vague answers are a warning.
Red flags
- A guaranteed "number one" position. Google's guide says no one can guarantee ad position.
- Claims that ads will improve your free Google ranking. Google forbids partners from saying this.
- Refusing to give you account access, or running ads in an account you do not own.
- Long lock-in contracts with no performance review point.
- Pressure tactics, or someone claiming to "be from Google" with a Gmail address.
The middle path
Many owners do best with a hybrid: a specialist builds the account, tracking and landing pages, then trains your team and reviews monthly. You keep control and learn. The specialist catches the costly mistakes. For a wider view of choosing help, see how to choose a medical marketing agency.

Frequently asked questions
How much does a Google Ads agency cost in Dubai?
Fees vary: a fixed monthly fee, a percentage of ad spend, or a mix. Always get the fee in writing and separate from your ad budget, which should be paid to Google from your own account.
Who should own the Google Ads account, me or the agency?
You. Create it under the clinic's Google account and link the agency's manager account. You can unlink an agency at any time and keep all your data and history.
What does Google Partner status mean?
The agency meets Google's minimums for account optimisation, spend across clients and staff certification. Premier Partner means top 3% in the country. It does not prove healthcare expertise.
Can I switch agencies without losing my campaigns?
Yes, if the account is yours. Unlink the old agency's manager account and link the new one. If the agency owns the account, ask for ownership transfer before you leave.
How many hours a week does Google Ads take?
For a small clinic account, plan a few hours a week in the first two months, then about an hour a week. More treatments and branches mean more time.
